Simple Translation
When one is in his final moments, flies will settle on top of wounds, which are full of pus. They will lay eggs on the wounds and the water, festering from those wounds, will flow. The flies will cause itching all over the physical form of the person, due to which, one would lose consciousness, being unable to tolerate the itching. Before reaching such a stage, if one were to recite “namO nArAyaNAya” with the mouth, cup the palms together in anjali pose, over the head [worshipping emperumAn], even if nithyasUris [permanent dwellers of paramapadham] and mukthAthmAs [those who have been liberated from samsAram and have reached paramapadham] are made as guarantors, they will not allow the person, who has reached paramapadham, to return to earth.

eeyinAl arippuNdu mayangi – itching will be caused by the flies; unable to tolerate that itching, one will lose consciousness.
ellai vAychchenRu sErvadhan munnam – before attaining the final moments of one’s life
vAyinAl namO nAraNA enRu – to realise the benefit of having been provided with a mouth. Saying “I do not belong to myself, I belong to you” is the benefit of having been provided with a mouth [when one says “I do not belong to you”, it is the equivalent of “nama:” and when one says “I belong to you”, it is the equivalent of saying “nArAyaNAya”, with these two terms coming from the second and final words of thirumanthram].
maththagaththidaik kaigaLaik kUppi – realising the benefit of having been provided with a head
pOyinAl – if one does these [reciting namO nArAyaNAya and worshipping emperumAn with the hands] and reaches mOksham, the benefit of these activities is to go through the path of archis [the path of light, which leads one to SrI vaikuNtam]. If the person performs anjali once [before his death], the benefit which he realises while going in the path of archis will be, as mentioned in periya thirumozhi 3.7.8 “maRRellAm kai thozhap pOy” (to be worshipped by everyone else) and in thiruvAimozhi 10.9.3 “thozhudhanar ulagargaL” (the residents of the higher worlds will join their palms together and perform anjali) that wherever he sees, there will be forests of anjali, performed by everyone on his way to SrI vaikuNtam. Even when he reaches SrI vaikuNtam, the benefit will be, as mentioned in mahAbhAratham Santhi parvam 344.45 “badhdhAnjali putAhrushtA:” (everyone will perform anjali, saying “nama:” joyously]. When the person surrenders the AthmA once [to emperumAn], those in SrI vaikuNtam will offer them thripAdhvibhUthi [the spiritual realm, which is three times as large as samsAram], as mentioned in thiruvAimozhi 10.9.6 “ANmingaL vAnagam AzhiyAn thamar” (You, who are the servitor of emperumAn, should rule over this place). When one says nama: once, the benefit will be to keep worshipping emperumAn, throughout the time, surrounding him, in SrI vaikuNtam, as mentioned thiruppallANdu 12 “namO nArAyaNAya enRu” (carrying out mangaLASAsanam to emperumAn, making him happy).
piNai kodukkilum pinnai iththisaikku enRum pOga ottArE – even if nithyasUris and mukthAthmAs were made as guarantors [for the person who has reached SrI vaikuNtam, to return to earth], they will not allow the person to return. The reason for that is that this place [samsAram] is such that it will make even one, who carries out servitude to emperumAn in SrI vaikuNtam, to become subservient to it. Also, emperumAn is one, who, after lifting a person up through a ladder, will remove the ladder [making it impossible for the person to return]. This emphasises the fact that there is no return as a result of one’s karma [past deeds] since all the past deeds get destroyed, before one reaches SrI vaikuNtam.
